Both plans are mandatory unless covered by spousal plan.

Extended Health Plan

Effective January 1, 2012

• Family Rate: $105.20 per month
• Single Rate: $52.60 per month

Dental Plan

Effective October 1, 2011

• Family Rate: $96.68 per month
• Couple Rate: $66.20 per month
• Single Rate: $43.08 per month
